Charles YaoandClaude Opus 5 ccb217d72f fix(dify-builder): a re-fix starts a fresh unknown-outcome count ...

Fix's re_fix action resets the rest of the repair loop's state but kept
unknown_outcome_count, so after the cap tripped the first unknown
outcome of the new repair cycle re-capped immediately instead of being
re-runnable once.
